FARS/CFARS

Participating Staywell and HealthEase (Medicaid) behavioral health providers in Florida are required to become certified FARS and/or CFARS raters. The FAR is for health plan members over 18 years of age and the CFARS is for health plan members 18 and under. The FARS/CFARS page provides information on requirements providers must meet for Staywell and HealthEase health plan members in Florida and includes FARS/CFARS forms in Adobe format that may be completed online, printed and faxed.
